Name: your name
Rank: your rank, if any
Connection: your connection to Mr. Deese, such as student, colleague, friend, relative, etc.
Dates: if your are or were a student of Mr. Deese, the dates you have been or were a student
Testimonial: remember that a testimonial states why you appreciate Mr. Deese and/or why you enjoying training at his school.
